HerdQuote is operated by Atlas Benefits, an independent insurance agency. When you submit a coverage request, we collect your name, ZIP code, phone number, email address, coverage interest, and the contact choices you make. We also record the date and wording of your permission, page and campaign source, and basic technical information needed to protect and track requests.

How we use your information

We use it to respond to your request, connect you with a licensed Atlas Benefits agent, keep a record of your permission, manage follow-up, and understand which HerdQuote campaigns work. We do not ask for diagnoses, medications, Social Security numbers, or payment details in this form.

Sharing and storage

Your request may be stored in HerdQuote's secure intake system and transferred to Atlas Agent CRM for agency follow-up. We do not sell, auction, or distribute the contact information you submit as a lead to other insurance agents or lead buyers. Your request is handled by Atlas Benefits, and an agent from our agency follows up about it. Authorized Atlas Benefits personnel and service providers that operate our systems may access information to provide this service. If you later choose to apply for coverage, information required for an application may be shared with an insurer or the Health Insurance Marketplace as part of that separate process. We may also disclose information when required by law.

Contact and choices

You may ask us to correct or delete your information or stop marketing contact by calling Atlas Benefits at 727-203-3513. If you opted in to texts, reply STOP to any text to opt out. We retain records as needed for service, legal, and compliance purposes.

This initial website records referral and campaign parameters submitted with your form. If analytics or advertising cookies are added later, this notice will be updated to describe them.
